We at Springbox Technologies LLC d/b/a (“Cleared,” “we,” “us,” or “our”) have created this privacy policy (this "Privacy Policy") because we know that you care about how information you provide to us is used and shared. This Privacy Policy relates to the information collection and use practices of Cleared in connection with our website located at www.getcleared.com (the "Website"), our telehealth service portal (“Telehealth Portal”) which is made available to you through the Website, and our products that are made available for purchase through our Website (“Products”).

DESCRIPTION OF USERS AND ACCEPTANCE OF TERMS

This Privacy Policy applies to visitors to the Website, who view only publicly-available content and/or purchase Products (“Visitors”), and individuals who have signed up to access and use our Telehealth Portal (“Registered Users”).

By visiting our Website and/or purchasing our Products, Visitors are agreeing to the terms of this Privacy Policy and the accompanying Terms of Use.

THE INFORMATION WE COLLECT/RECEIVE

In the course of operating the Website and the Telehealth Portal, Cleared collects and/or receives the following types of information.

  1. Contact Information

    We use sign up to use our Telehealth Portal or contact us through the “Contact Us” feature on our Website, you will be asked to provide us with your contact information, which may include your first name, last name and email address (“Contact Information”). The Contact Information is used to provide the requested service or information and to contact you for purposes of direct marketing of our current and future services.

  2. Online Consultation Information

    When you sign up to use our Telehealth Portal, you will be asked to complete a questionnaire to provide us with some basic identification information and background health information that relates to your past, present, or future physical or mental health or condition, including but not limited to, current health condition, and medications you take (collectively, “Online Consultation Information”).

  3. Billing Information

    In order to use our Telehealth Portal and/or purchase our Products, you will be required to provide certain additional information which may include a credit card number, expiration date, billing zip code, activation code, bank information, and similar information (“Billing Information”). Such Billing Information will be collected and processed by our third-party payment vendor pursuant to the terms and conditions of their privacy policies and terms of use. Cleared does not directly obtain or process any Billing Information.

  4. Order Information

    When you purchase Products through our Website, we will also ask you to provide certain additional information, including but not limited to, the Products you would like to purchase, and the shipping information (collectively, the “Order Information”).

  5. Other Information

    In addition to the Contact Information, Order Information, and the Online Consultation Information, we may collect or receive the following information:

    • From Your Activity. In an ongoing effort to improve the Website and the Telehealth Portal we automatically collect certain information when you visit the Website and use our Telehealth Portal. This information consists of IP addresses, browser type and language, referring and exit pages and URLs, date and time, amount of time spent on particular pages, what sections of the Website you visit, and similar information concerning your use of the Website and the Telehealth Portal.

INFORMATION COLLECTED BY OR THROUGH THIRD-PARTY ADVERTISERS/REMARKETERS

We may share Other Information about you with third parties, including, but not limited to, advertising and remarketing providers, or other brand partners, for purposes of personalizing or otherwise understanding how you engage with ads or other content. HOW WE USE AND SHARE YOUR INFORMATION

You authorize use to share your Contact Information and Online Consultation Information with the healthcare providers that you connect with through our Telehealth Portal. You acknowledge and agree that the information made available to such healthcare providers is not subject to the terms of this Privacy Policy. You may revoke all or part of such authorization granted to Cleared in writing at any time by sending a signed and dated statement to Cleared at info@getcleared.com.
You authorize us to use the Contact Information, Order Information, Online Consultation Information, and Other Information (collectively, the "Information") to provide the Website, the Telehealth Portal, and the Products to you, solicit your feedback, inform you about our products and services and those of our third-party marketing partners, and to improve the Website and the Telehealth Portal.
We may also use and share the Information as described below.
  • Agents, Providers and Related Third Parties. We may engage other companies and individuals to perform certain business-related functions on our behalf. Examples may include providing technical assistance, customer service, and marketing assistance. These other companies will have access to the Information only as necessary to perform their functions and to the extent permitted by law. We may also share your Information with any of our parent companies, subsidiaries, or other companies under common control with us.

If you have signed-up to use the Telehealth Portal and/or purchased our Products, you may access, review, and make changes to your personal information by following the instructions found on the Website or by emailing us at info@getcleared.com. In addition, you may manage your receipt of marketing and non-transactional communications by clicking on the "unsubscribe" link located on the bottom of any Cleared marketing email. You cannot opt out of receiving transactional e-mails related to your account. We will use commercially reasonable efforts to process such requests in a timely manner. You should be aware, however, that it is not always possible to completely remove or modify information in our subscription databases.

PRIVATE MESSAGING

You may be able to communicate with your healthcare providers via direct messages sent through the Telehealth Portal. If you choose to contact a healthcare provider outside of the Telehealth Portal, you acknowledge and agree that none of the information contained in that message (including, without limitation, personal Information and/or health information) is covered by this Privacy Policy and we are not responsible or liable in any way for how the health care providers use that information or for any breaches of personal information during the transmission of the message.

HOW WE PROTECT YOUR INFORMATION

Cleared takes very seriously the security and privacy of the personal information that it collects pursuant to this Privacy Policy. Accordingly, we implement reasonable and appropriate security measures to protect your personal information from loss, misuse and unauthorized access, disclosure, alteration and destruction, taking into account the risks involved in processing and the nature of such data, and comply with applicable laws and regulations. We cannot guarantee the security of our databases or the databases of the third parties with which we may share your information (as permitted herein), nor can we guarantee that the information you supply will not be intercepted while being transmitted over the Internet. CHILDREN

We do not knowingly collect personal information from children under the age of 18 through the Website and the Telehealth Portal. If you are under 18, please do not give us any personal information. Our servers are located in the US. The Website and the Telehealth Portal may contain links to third-party websites (“External Sites”). Cleared has no control over the privacy practices or the content of any such External Sites. CHANGES TO THIS PRIVACY POLICY

This Privacy Policy is effective as of the last updated date stated at the top of this Privacy Policy. We may change this Privacy Policy from time to time, and will post any changes on the Website and the Telehealth Portal as soon as they go into effect. By accessing the Website, and/or purchasing Products after we make any such changes to this Privacy Policy, you are deemed to have accepted such changes if you are a Visitor. Registered Users will be required to affirmatively accept the revised Privacy Policy prior to next logging-in to their account.
